To experience a fractured neck of femur following a fall is one of the most traumatic experiences and one that carries significant risks – such as a 10% mortality rate nationally, a 20-30% chance of not being able to return to your place of residence and high co-morbidity ratios.
Effective clinical partnerships can transform the way in which clinical care is provided, improve clinical outcomes, release significant cost savings and/or deliver additional income.
We have over 20 years experience of designing new clinical pathways and of benchmarking current pathways against national best practice models of care, providing the gap analysis and then working with the in-house staff to implement the improvement plans and close the gap.
Our work within this field is varied, but specifically within the pathway of fractured neck of femur, we:
- Work with the local clinical teams to understand the current pathway and establish the real costs.
- Work with the financial management teams to understand the income received for this cohort of patients.
- Ensure all activity is appropriate, correctly coded and develop a system to robustly track this income specifically.
- Develop senior managers and clinical leaders to lead the cultural, organisational and service changes.
- Develop and agree key performance indicators for the individual, the department and the organisation to track success and monitor performance.
- Support the implementation and change management process.
- Work to deliver, maintain and hold the clinical, operational and financial gains.
- Our experience in one Trust evidenced an annual £1.3m revenue cost reduction by moving the fractured neck of femur pathway to best practice, significantly reducing length of stay, releasing Consultant PA sessions by redesigning the on-call trauma rotas and by ensuring all income was received appropriately by redesigning an electronic discharge and coding form.
We have a team of professionals with a strong Health Service background who have many years experience in scoping and delivering redesigned clinical pathways and we would welcome an opportunity to discuss this further with you.
